Hmmm... i still say locke. He is the only one we know of with a confirmed spine problem. I doubt its jack, since the promo showed right after the episode showed jack talking to whoever it is who has the tumour... and locke was in the promo a lot.
as for how or why they would have lock'e x- rays... don't they take a certain interest in all the losties? They would have it the same way/reason they had a whole file full of info on jack, his family, everything.
as for playing the irony card, what would be more ironic than the man of science operating on the man of faith?
